We accept payment from most insurance companies. Some of the popular ones in our area that we accept are:

• Blue Cross/Blue Shield

• Health First Health Plans

• United Healthcare

• Cigna

Our office will verify your insurance for you on your first visit. However, if you wish to know ahead of time you may download this form, which gives you the same questions we ask the insurance company to determine your financial share.

We also accept & file auto injury/PIP insurance and worker's compensation insurance.

Non-Insured

We pride ourselves on making care affordable for every budget. We have several payment options you may use. These include:

• A budgeted weekly payment plan with our office throughout the extent of your treatment plan.
• Pay each visit at you go (our rates are kept affordable).
• We accept Cash, Personal Check, Visa, MasterCard and Discover.

We also accept payment with CareCredit. This allows you to finance your entire treatment plan for up to 12 months with NO INTEREST! You may go to www.CareCredit.com to apply. You will receive an instant decision with your new account number, if approved.

Healthcare Payment Solutions (HCPS) is another option, for longer term payments. If interested, call the office for more information on HCPS!
